Instructions for form completion
- Complete the form.
- Proofread the form and ensure the information is correct.
- Type your name in the signature line. (If submitting via fax or in person, actual signature will be required.)
- Save the form. From the file menu, select SAVE AS. Select a location on your computer or flash drive and click the SAVE button.
- Print the form for fax or in-person delivery.
E-Mail Submission of Form to Professor
- Type EXPLANATORY STATEMENT FOR ABSENCE FROM CLASS in the subject line of your e-mail.
- Enter any text you believe is relevant in the body of the e-mail.
- Attach the previously saved form.
- For your own record of submission, include your e-mail address on the CC line.
