Instructions for form completion

  1. Complete the form.
  2. Proofread the form and ensure the information is correct.
  3. Type your name in the signature line.  (If submitting via fax or in person, actual signature will be required.)
  4. Save the form.  From the file menu, select SAVE AS.  Select a location on your computer or flash drive and click the SAVE button.
  5. Print the form for fax or in-person delivery.

 

E-Mail Submission of Form to Professor

  1. Type EXPLANATORY STATEMENT FOR ABSENCE FROM CLASS in the subject line of your e-mail.
  2. Enter any text you believe is relevant in the body of the e-mail.
  3. Attach the previously saved form.
  4. For your own record of submission, include your e-mail address on the CC line.
